Electromyographic and endocrine studies in chronic thyrotoxic myopathy.

چکیده
If the number of cases of a disease reported in the literature can be taken as an index of frequency, chronic thyrotoxic myopathy is a very rare disease. Its interest lies, however, not so much in its rarity as in the light it may throw on the inter-relationship of the endocrine glands and muscle function. In a previous paper (Sanderson and Adey, 1949) some aspects of this problem were reviewed and a case was reported in which the electromyographic findings were those typical of muscle dystrophy. Two further cases are presented here with the electromyographic findings, and with investigation of the function of the adrenal cortex in one case. The electromyograms give positive evidence of a muscular dystrophy of severe degree and show a return to the normal pattern after treatment of the thyrotoxicosis, thus making the diagnosis beyond doubt. The tests of adrenal cortical function were designed in rather a hurry and partly for this reason, but more especially because such tests are comparatively crude, the evidence for depressed function is inconclusive. That such a depression of function may occur in this disease is suggested by the necropsy findings of Thorn and Eder (1946) of atrophy of the glomerular layer of the adrenal cortex.

Evidence suggests that most hyperthyroid patients have a proximal myopathy. The more severe this is the more frequently are distal muscles, and ultimately, bulbar muscles involved. Probably acute thyrotoxic myopathy or encephalopathy supervenes on a previous chronic background or occurs concurrently with skeletal muscle involvement. Using careful electromyographic techniques evidence of myopath...

OBJECTIVE To document electromyographic (EMG) changes in thyrotoxic patients, and to categorize the type of myopathic process in thyrotoxicosis. METHODS This case control study was designed to show the EMG changes in thyrotoxic patients and to compare these findings with that of normal aged matched controls to show the significance of these changes in thyrotoxic patients. Student`s test was a...
متن کاملPREVALENCE OF HYPERCALCEMIA AND HYPERCALCIURIA IN THYROTOXIC PATIENTS OF A REFERRAL ENDOCRINE CLINIC
Many patients with thyrotoxicosis develop hypercalciuria and nypercalcemia. Urine calcium excretion depends on diet, latitude and sunshine exposure. With regard to variation of these indices in different geographical regions, we conducted a one-year study on 202 patients, 65 males and 137 females, with thyrotoxicosis of varying severity. The mean age of patients was 35.24±11.95 and 35.87±1...

A variety of neuromuscular disorders has been described in adults in association with overactivity of the thyroid (Millikan and Haines, 1953; Ramsay, 1966; Engel, 1972). The most common of these disorders is a chronic myopathy, characterized by muscular atrophy and weakness involving predominantly proximal muscles (Adams and Rosman, 1971). The myopathic symptoms may precede the symptoms of thyr...
متن کاملA Case of Thyrotoxic Myopathy with Extreme Type 2 Fiber Predominance
In hyperthyroidism, many patients had neuromuscular symptoms and clinical weakness correlated with free thyroxine (T4) concentrations. The common clinical symptoms of chronic thyrotoxic myopathy were characterized by progressive weakness in proximal muscles and atrophy. A 55-year old woman was visited our hospital with two years of progressive weakness of both legs. Physical examination showed ...
